How Fax-Cover Pages are Populated

Fax cover pages are automatically populated when sending a fax via Sfax.  The information, shown below is pulled from several places within the application.

Company address (2nd line; untitled) $Sender. AddrLine2And3$ Settings > Manage Account > Basic Settings  > Address (2nd and 3rd lines)
Company address (3rd line) CityStateZip) $Sender.CityStateZip$ Settings > Manage Account > Basic Settings > City, State, Zip code 
Company address (4th line) (website) $Sender.Website$ Settings > Manage Account > Basic Settings > Website
To: $Recipient.Name$ Recipient's name entered/chosen on Fax send (Populates by fax number, if manually entered)
Company: $Recipient.CompanyName$ Recipient's company name entered/chosen on Fax send (Does not display if no company name exists)  
Fax: $Recipient.FaxNumber$ Recipient's fax number entered/chosen on Fax send
Subject: $Fax.Subject$ Text enter in the Subject line of the fax composition page
Ref.: $Fax.Reference$ Text enter in the Ref line of the fax composition page
From: $Sender.UserName$ First and last names of the user sending the fax
Fax: $Sender.FaxNumber$ The outgoing number selected on the fax composition page
Phone: $Sender.PhoneNumber$ Settings > Manage Account > Basic Settings -> Phone (By default,  this field will auto-populate with the  Sfax-account phone number; but the administrator may change this to populate by the user's phone number)
Date: $Fax.Date$ Date stamp of the fax submission
Time: $Fax.Time$ Time stamp of the fax submission
Time Zone: $Fax.TimeZone$ Time zone of user sending the fax
Pages: $Fax.PageCount$ Total number of pages (including cover page) in the sent fax
Remarks: $Fax.Remarks$ Remarks entered on the fax composition page
